Welcome to our New and Established Patients!We welcome you to the UCSF Multiple Sclerosis Center. In this section, you will learn about becoming a new patient, your ongoing care, MRIs and other testing.
Reminders for your upcoming visitMany insurance plans require a referral from your primary care physician before you can schedule a visit with us. Please contact them to check on this.
Be sure to have any previous neurologists fax or mail your medical records, including MRI films, to us before the date of your visit. On the day of your visit, be mindful of our on-time protocol and bring your insurance card.
If you need to do so, please be sure to cancel your appointment as early as possible so we can offer that doctor’s time to another patient.
To ensure that we have your updated medical information, please fill out one of the forms below and bring it with you to your appointment.

Sending Radiology ImagesTo send your Radiology Images securely via the web (ReferralBox) to our UCSF Department of Radiology Imaging Library, please read this document and then ask our clinic to send you an email notification.
MRI's and Ancillary TestingFor general questions about what is needed to schedule MRIs and other “ancillary tests”, please call (415)353-2069.
