We welcome you to the Multiple Sclerosis (MS) Center at UCSF Medical Center. In this section, you will learn about becoming a new patient, your ongoing care, managing prescription refills, MRIs and other testing, and disability benefits.

New Patients

Please be aware that many insurance plans will require a referral from your primary care physician before you can schedule a visit with a MS specialist at UCSF. Please contact your insurance to learn whether you need to obtain an authorization prior to your visit. Be sure to have previous neurologists fax or send your medical records, including MRI films, to us before the date of your visit. When you arrive for your visit, please arrive 15 minutes early to ensure time to complete paperwork. Please be sure to bring your insurance card to all visits.

Please be sure to bring your insurance card and any authorization your insurance has sent you. If you need to cancel an appointment, please do so as early as possible so we can offer the appointment to another patient.

To assist us in insuring that we have necessary and up-to-date medical information, please download, print, and fill out the appropriate form before your appointment. These forms require detailed information, some of which may be most easily completed when you're at home. They should take 20 to 30 minutes to complete. Please complete them before your appointment.

Most disease modifying therapies for MS require preauthorization. Please be sure to contact MS Center nursing staff at least one month before your prescription expires, to obtain authorization.

Please note- you must have seen your MS provider within the past year in order for us to faciliate your medication refill request.

Disability Benefits

There are different types of disability insurance which may be applicable depending upon the severity of the illness. It is important to know what you are eligible for so you can plan for the future and minimize future stress.

Disability benefits generally are either short-term or long-term in nature and are administered by private insurance groups or government programs. Group disability can be purchased on your own or through your employer's insurance company. It can be short- or long-term. Generally these policies stipulate that you will receive a percentage of your present salary for a specific amount of time.

Please contact our office at (415)-353-2069 to set up an appointment to fill out any paperwork related to your disability claim.
